  • Completed Valet Parking Questionnaire
  • Completed employee list including – name, date of birth, drivers license number, title/position, Individuals furnished a vehicle for personal use, payroll
  • 4 years of prior losses
  • Details of owner experience and valet attendant hiring practices
  • Garage Valet Parking Supplemental Application
